Welcome to Acupuncture & Holistic Health Associates. By scheduling an appointment or using our services, you agree to these Terms of Service. Please read them carefully before your first visit.

Acceptance of Terms

By accessing our website, scheduling appointments, or receiving treatment at Acupuncture & Holistic Health Associates, you acknowledge that you have read, understood, and agree to be bound by these Terms of Service and our Privacy Policy. If you do not agree to these terms, please do not use our services.

Services Provided

Acupuncture & Holistic Health Associates provides complementary and alternative healthcare services, including but not limited to:

  • Acupuncture treatments
  • Traditional Chinese Medicine consultations
  • Herbal medicine recommendations
  • Cupping therapy
  • Nutritional guidance and lifestyle counseling
  • Other holistic health modalities

Our services are designed to complement, not replace, conventional medical care. We encourage you to maintain regular communication with your primary care physician and other healthcare providers.

Patient Responsibilities

Accurate Information

You agree to provide complete and accurate health information during your initial consultation and to update us about any changes to your medical condition, medications, or health status. Failure to disclose relevant health information may affect the safety and effectiveness of your treatment.

Following Treatment Plans

While we will provide personalized treatment recommendations, you are ultimately responsible for your own healthcare decisions. We encourage open communication about your treatment plan and any concerns you may have.

Communication

You agree to promptly notify us of any adverse reactions, changes in your condition, or concerns about your treatment. We maintain regular office hours and emergency contact protocols outlined in your patient materials.

Appointment Policies

Scheduling and Confirmation

Appointments can be scheduled by phone, email, or through our online booking system. You will receive a confirmation of your appointment via your preferred contact method. Please arrive 10-15 minutes early for your first appointment to complete necessary paperwork.

Cancellation Policy

We understand that schedules change. If you need to cancel or reschedule your appointment, please provide at least 24 hours' notice. Cancellations made with less than 24 hours' notice may be subject to a cancellation fee of 50% of the session cost.

No-Show Policy

Failure to attend a scheduled appointment without prior notice will result in a charge equal to the full session fee. Repeated no-shows may result in a requirement to prepay for future appointments or termination of the patient-practitioner relationship.

Late Arrivals

If you arrive more than 15 minutes late for your appointment, we may need to reschedule to ensure adequate treatment time and avoid disrupting other patients' appointments. You may be subject to our cancellation policy in such cases.

Payment Terms

Payment Methods

We accept cash, checks, and major credit cards (Visa, MasterCard, American Express, Discover). Payment is due at the time of service unless other arrangements have been made in advance.

Insurance

We are in-network providers for several insurance plans and can provide superbills for out-of-network reimbursement. Please verify your coverage before your appointment. You are responsible for any portions not covered by insurance, including deductibles, co-pays, and non-covered services.

Pricing

Our current fee schedule is available upon request and may be updated periodically. We will notify you of any changes to pricing that may affect your care. Package deals and discounts may be available for multiple sessions paid in advance.

Outstanding Balances

Accounts with outstanding balances over 30 days may be subject to a monthly finance charge of 1.5% (18% APR). Accounts over 90 days past due may be sent to collections, and you will be responsible for all collection costs and legal fees.

Informed Consent and Treatment

Before beginning treatment, you will be asked to sign an informed consent form acknowledging that you understand:

  • The nature of acupuncture and holistic treatments
  • Potential benefits and risks
  • Possible side effects and complications
  • Alternative treatment options
  • Your right to refuse or discontinue treatment at any time

While serious side effects from acupuncture are rare, minor side effects may include temporary soreness, minor bleeding or bruising at needle sites, or temporary aggravation of symptoms.

Scope of Practice and Limitations

Our practitioners are licensed acupuncturists and traditional Chinese medicine practitioners. We do not diagnose or treat conditions outside our scope of practice. We do not:

  • Prescribe pharmaceutical medications
  • Perform surgery or invasive procedures
  • Provide emergency medical care
  • Replace necessary conventional medical treatment

In cases requiring medical attention beyond our scope, we will refer you to appropriate healthcare providers.

Confidentiality and HIPAA Compliance

We are committed to protecting your health information in accordance with HIPAA regulations. Your medical records and personal information will be kept confidential except as required or permitted by law. Please see our Privacy Policy for complete details on how we handle your information.

Minors and Consent

Treatment of patients under 18 years of age requires consent from a parent or legal guardian. The parent or guardian must be present during the first visit and must sign all consent forms on behalf of the minor.

Intellectual Property

All content on our website, including text, graphics, logos, images, and educational materials, is the property of Acupuncture & Holistic Health Associates and is protected by copyright laws. You may not reproduce, distribute, or use any content without our written permission.

Website Use and Disclaimer

Professional Conduct

We maintain professional boundaries and expect the same from our patients. Inappropriate behavior, harassment, or disrespectful conduct toward staff or other patients will not be tolerated and may result in termination of care.

Termination of Services

Either party may terminate the patient-practitioner relationship at any time. If we terminate the relationship, we will provide you with reasonable notice and assistance in finding alternative care if requested. You may discontinue treatment at any time but remain responsible for payment of services already rendered.

Dispute Resolution

In the event of a dispute arising from these Terms of Service or your treatment, we encourage resolution through direct communication. If a dispute cannot be resolved informally, both parties agree to attempt mediation before pursuing legal action.

These terms shall be governed by the laws of the State of Wisconsin. Any legal action must be brought in the courts of Milwaukee County, Wisconsin.

Changes to Terms

We reserve the right to modify these Terms of Service at any time. Changes will be posted on our website with a revised "Last Updated" date. Continued use of our services after changes are posted constitutes acceptance of the modified terms.

Severability

If any provision of these Terms of Service is found to be unenforceable or invalid, that provision shall be limited or eliminated to the minimum extent necessary, and the remaining provisions shall remain in full force and effect.
